Registering online with Patford House Partnership is quick and easy! You do not need proof of address or immigration status, ID or an NHS number. The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it faster and easier for you to register.

Register as a Patient of Patford House Partnership
We are always happy to welcome new patients to Patford House Partnership.
To join PHP, simply complete the online form above or complete our registration form and patient questionnaire that can be downloaded using the above links. Alternatively pick up a registration pack at any PHP branch. New patients will be registered with Patford House Partnership, meaning that they can access services from all three of our Surgeries when needed.
Our friendly Receptionists will be happy to answer any questions you may have and will allocate you a dedicated Registration Champion who acts as your point of contact throughout the registration process and will also contact you to welcome you to the Practice once your application is complete.
Completing an application on-line or bringing in your forms to our surgeries will initiate your registration request. You may be asked to visit us at any of our branches to provide proof of ID as part of the registration process.
Please note that applications for registration have to be approved and refusal of applications may occur for example if inadequate information is provided on the registration form, you are out of our Practice Boundary or if you are a serving member of the armed forces.
Please allow up to three weeks to process registrations. This means we have time to obtain your full medical record so we can provide you with the best care and treatment. We cannot offer patients any routine appointments before your registration has been processed, but if you do have any urgent health needs please make our team aware of this as soon as possible.
