SystmOnline is available for all patients of Patford House Partnership. In order to access your account, a username and password will need to be obtained from the PHP Patient Services team.  All newly registered patients are provided with their SystmOnline account details upon registering with the Practice. If you do not have a username and password you can contact the Patient Services team today via 01249 821831, speak to one of our helpful Receptionists at any of our branches  or message us at BSWCCG.PHP@nhs.net. Your account details will be sent electronically to the contact details you have verified on your health record. We cannot give usernames and passwords over the phone for confidentiality reasons.
