First Contact Physios (FCP)

A First Contact Physiotherapist is a trained and qualified Physiotherapist registered with the Health and Care Professions Council who has enhanced skills and work at the top of their field.

They can:

Help patients with musculoskeletal issues such as back, neck and joint pain

Assess and diagnose musculoskeletal and joint pain issues

Provide a plan of care and follow up on treatment effectiveness

Refer patients to the wider MSK team and arrange further tests (such as X-rays)

You may be offered a telephone consultation with the FCPs for your initial assessment.  This helps the FCPs to make sure you get the right care and treatment as swiftly as possible.

Mental Health Nurse Specialist (PCN)

A Mental Health Nurse is a registered nurse who’s undertaken additional training, giving them the knowledge and competencies needed to help patients with low mood or diagnosed mental health issues.

Following an initial assessment, the mental health nurse will offer short-term interventions which may include anxiety management, support with depressed mood, advise on possible medication options and liaise with the GP to agree care plans.

This role can also refer patients into the wider Mental Health service including Phsycological Therapies (IAPT) or to AWP (Avon Wiltshire Mental Health Service)

We share our Mental Health Nurse with Northlands Surgery which means you may be offered an appointment at Northlands to be seen - this helps to provide more access to appointments for our patients.

Advanced Practitioner - (Paramedic)

A paramedic or advanced (emergency care) practitioner is a registered paramedic who’s completed additional training, enabling them to:

Carry out home visits

Undertake face-to-face and telephone triage

Manage patients with common illnesses or minor injuries

Advise patients on healthcare matters including the self-management of conditions

Our Advanced Practitioner, Paul Draper, helps to provide routine care to our patients who are aged over 65.  He also works alongside our care coordinators to carry out welfare assessments as a home visit.
